About Vivent Health:
Rooted in the shared commitment of our legacy AIDS service organizations to fight to end HIV/AIDS, Vivent Health strives to achieve health justice for people living with HIV by providing lifesaving healthcare and critical social services tailored to each person we serve. We have advanced equitable health outcomes by supporting our local communities through awareness, education, fundraising, prevention, and advocating for policies that ensure everyone impacted by HIV can live their healthiest life. Our innovative approach to integrated HIV care and treatment enables our patients to exceed the national benchmarks for viral load suppression.
Vivent Health in Austin currently serves nearly 2,700 people living with and vulnerable to HIV and sexually transmitted infections, as well as countless people at risk for opioid overdose. We also have clinics in Denver, CO; Chicago, IL; St. Louis and Kansas City, MO; and, across 10 cities in Wisconsin. Learn more at www.ViventHealth.org.
Our Proven Model of Care
Vivent Health’s integrated, team-based structural health intervention model of care is proven to be effective because it is rooted in the understanding that HIV cannot be treated without addressing barriers to care such as food insecurity, housing instability, and other chronic health conditions. Rather than providing a one-size-fits-all solution, each of our patients has a multi-disciplinary care team that provides the physical, behavioral, pyschosocial, and social services to assure their individual needs are addressed, care is closely coordinated, and the best clinical outcomes are achieved.
95% of Vivent Health’s patients achieve viral suppression, exceeding the U.S. Department of Health and Human Services’ 2030 Ending the HIV Epidemic goal of 90%, national rate of 66%, and Austin rate of 64%.
93% of Vivent Health’s Black/African American patients achieve viral suppression, which is significantly higher the national rate of 62%, and Austin rate of 59%.
